St. Andrew Catholic School students participate in many wonderful curricular, co-curricular, and extra-curricular activities.  In order to spread the good news about school, photos are regularly taken of our student at work and play.  

We wish to have your permission to include your child's photograph, first and last name, and/or school work in these projects.

This form must be completed for every student.  If you have multiple children enrolled, please complete this form for each child.

CONSENT FOR PUBLICATION

I give St. Andrew Catholic School my consent to use my child's photograph, first name, and/or school work in all school related publications, including, but not limited to: school yearbook, newsletters, parish bulletins, website, in-house publications, and marketing materials without restriction.

I understand that photographs, first and last name, and/or school work may also be released to our community newspapers, such as Angelus News, the Outlook, Pasadena Now, and/or Pasadena Star News.

If you answered YES, but with restrictions, please specify which option you are NOT giving consent to.

PARTIAL RESTRICTIONS 

If you would like to opt-out of having your child's photo, first name, and/or school work from being used in any media, please indicate by checking one or all of the following selections

I hereby waive, release, and forever discharge any and all claims, demands, or causes of action against St. Andrew Catholic School and its faculty, staff, employees, agents, contractors, and any other person, organization, or entity assiting them in connection with publishing in other media, for damages or injuries in any way related to, connected to, or arising from the publishing or the use of that information, and expressly assume the risk of any injury or damage resulting from said publishing.

I acknowledge that the school's website is not private and can be reviewed, copied, downloaded, and transmitted by anyone with access to the internet and that the school has no control over this.  I hereby waive release and forever discharge any and all claims, demands or cause of action against St. Andrew Catholic School and its faculty, staff, employees, agents, contractors or any other person, organization or entity assisting them in the connection wit the posting information on the website for damages or injuries in any way related, connected to, or arising from the publishing or posting of information on the school's website or the use of that information and expressly assume the risk of any injury or damages resulting from said posting infomation on the website.

I further understand and agree that this authorization remains in effect until such time as it is withdrawn in writing. 

I understand that if I change my relating to this authorization, that I will submit another authorization form to the school
